Diabetes Self-Management Education (DSME)
Our Mission is to be a Patient-Centered Diabetes Center for Excellence by providing culturally diverse Diabetes Self-Management Education with the goal to empower the patient, to optimize health and improve quality of life.

The Education

We offer an interactive, ongoing process involving the person with diabetes or the caregiver or family with a diabetes educator. The education aims to achieve optimal health status, better quality of life and reduce the need for costly health care.

Diabetes Statistics

  • Total: 25.8 million children and adults in the United States – 8.3% of the population – have diabetes.
  • Diagnosed: 18.8 million people
  • Undiagnosed: 7.0 million people
  • Pre-diabetes: 79 million people*
  • New Cases: 1.9 million new cases of diabetes were diagnosed in people aged 20 years and older in 2010

CAHN’s Diabetes Self-Management Education (DSME) Program** includes the following services:

  • Individual diabetes education sessions with a Registered Nurse and/or a Certified Diabetes Educator
  • Individual diabetes management plans
  • Access to diabetes resources, medications and testing supplies
  • Quarterly patient newsletters

*One-on-One sessions requires a referral from your Primary Care Provider
**CAHN’s DSME program is recognized by the American Diabetes Association

DSME Group Classes

  • Group classes are taught by a Registered Nurse and/or a Diabetes Peer Educator.
  • Topics of discussion include insulin administration, carbohydrate counting, meal planning and diabetes complications
  • Group cohorts are 10 weeks long and provide interactive education regarding diabetes
  • Cooking Demonstrations
  • Exercise instruction by a qualified instructor
  • Quarterly patient newsletters
  • Graduation ceremony
  • Peer support and follow-up

Mental Health Program

  • Assists diabetic patients with issues of coping and depression that can often be associated with diabetes
  • Provided by qualified mental health professionals
