The Wellness Bilingual Non-Medical Case Manager provides case management for HIV+ positive patients at Capital Area Health Network who receive services through the Ryan White Part-B funded Wellness Program, and fluently speaks spanish.

Non-Medical Bilingual Case Manager 

The Certified Medical Assistant is a graduate of an accredited school of Medical Assisting and certified accordingly. The Medical Assistant practices within their scope as directed by the agency Medical Director and the American Association of Medical Assistant (AAMA). The Medical Assistant provides evidence-based practice as per the Nurse Practice Act and other governing bodies and demonstrates growth in ability to care for patients with chronic diseases; requires consultation with experienced clinicians and benefits from feedback; reports directly to the Lead Licensed Practical Nurse and ultimately to the Nurse Managers.

Certified Medical Assistant 

Performs physical examinations of new and existing patients and  determines x-ray examinations and clinical laboratory tests required. Interprets examination findings, test results, and implements treatment
plans. Prepares and reviews case histories and obtains data through interviews. Provides continuing, comprehensive health maintenance and medical care to the entire family unit, to include preventative medicine, behavioral sciences, and community health.Participates in surgical, obstetrical, gynecological, and critical care procedures according to training and demonstrated ability. Determines need for consultation and assists in medical care treatment provided at the direction of other specialists

Functions under the collaborative practice agreement with a physician, provides professional medical services in the community health center setting. Provides comprehensive examination, screening, health education, and treatment of patients within the nurse specialist’s scope of practice using a systematic approach to history and physical appraisal of the patient.
Examines the patient, performs comprehensive physical examination, and
compiles patient medical data, including health history and results of physical examination.

Physician Assistant 

Assist Hiring Managers with the creation of job descriptions. Conduct intake session with all Hiring Managers. Source candidates for all open positions. Post job openings with approved job descriptions on Indeed, NOMAD and CAHN website. Schedule job interviews and assist in the interview process. Extend job offers to selected candidates and ensure background checks, drug screens and reference checks are completed.

Recruiter/Onboarding Specialist

The Licensed Practical Nurse is a graduate of an accredited school of nursing and is licensed to provide safe patient care as well as highly skilled nursing care under the supervision of a Physician and/or Registered Nurse. The Licensed Practical Nurse provides a higher level of evidenced-based practice as per the Nurse Practice Act and other governing bodies and demonstrates strong leadership and efficient skills in caring  for patients with chronic diseases; requires consultation with advanced practice
clinicians and benefits from feedback; reports directly to the Nurse Manager.

Licensed Practical Nurse

Position will consist of managing a full-time clinical caseload per week.
Complete comprehensive intakes as well provide outpatient services in the
treatment of mental health disorders while coordinating patient care with
medical professionals. Competence at intake assessment, diagnosis/treatment, rapport building, and ethical therapeutic techniques. Strong verbal and written communication skills are required.
Coordinate with other healthcare providers, agencies and community
resources in order to provide highest quality of care.

Provide continuous and comprehensive services necessary for the promotion of optimal mental health, prevention and treatment of psychiatric disorders and health maintenance. This includes the assessment, diagnosis, and management of mental health problems and psychiatric disorders. The NPP works collaboratively with other on site medical and mental health providers.
